Forms and New Patient Information
If you are visiting us for the first time, please arrive for your appointment at least 15 minutes prior to your scheduled time. This will allow you sufficient time to fill out necessary paperwork and for the staff to complete the computer registration process. Please review our Covid policy update.
What To Bring To Your 1st Visit
- A parent/guardian’s photo ID (This is to protect your child’s medical record information.)
- Your child’s current insurance card
- Your child’s immunization record
