Both Calvary Pediatrics and Sunshine Pediatrics have been awarded a prestigious award of Patient Centered Medical Home Level 2!! What does this mean? The medical home model holds promise as a way to improve health care in America. There are 5 attributes of a Patient Centered Medical Home:
- Comprehensive Care- this means we are accountable for meeting the large majority of each patient’s physical and mental health needs ( prevention, acute care, chronic care).
- Patient Centered- relationship based with an orientation of the WHOLE person. We partner with our patients and their families by understanding and respecting each patient’s unique needs, culture, values, and preferences.
- Coordinated Care- this means we coordinate your care across all aspects of the health care system by referrals to specialty care offices, home health care, and community services and supports.
- Accessible services- We strive to offer accessible services with shorter waiting times for urgent needs, enhanced in person hours, round the clock telephone or electronic access to a member of the team. - Quality and Safety- We have commitment to quality and quality improvement by ongoing engagement in activities such as using evidence based medicine and clinical decision support tools to guide shared decision making with patients and families, engaging in performance measurement and improvement and various other options.

Calvary Pediatrics is now a Reach Out and Read site!!!! We will be giving away books for children from 6 months to 5 years old to encourage the love of learning!
